Make sure your staff knows about these. You may use the previous versions through the end of March, but you are required to use the new versions beginning April 1.  **Upcoming Changes for these forms effective April 1, 2020** - **Important Message from Medicare (IM)** – Form CMS-10065. If the patient has Medicare Advantage insurance, the hospital will need to complete the contact information of the payer prior to giving to the patient. - **Detailed Notice of Discharge (DND)** – Form CMS-10066. The DND requires additional information to be entered by the hospital.   - **Medicare Outpatient Observation Notice (MOON)** –Form CMS-10611. The MOON form has not changed except for the expiration date.   **Transitioning to 2021 E/M Guidelines** Start planning now for the upcoming changes to E/M codes. Physicians, office staff, and coders will all need to prepare.
